Scott Conard, MD, FAAFP
Dr. Conard offers a unique perspective as a physician, entrepreneur and corporate executive. He is a seasoned professional with clinical expertise and a history of getting things done. His experience includes work in strategic planning, creation, development, and leadership of healthcare organizations, organizing physicians and health systems for a 500+ physician medical group and designing processes and systems to deliver streamlined care for integrated physician systems.
Michael Tuggy, MD, FAAFP
Michael Tuggy, MD is a Clinical Professor at the University of Washington School of Medicine, and a rural family physician and former medical director for Confluence Health in the mountain valley in Winthrop, Washington. He served for 7 years on active duty in the US Army including service in Iraq and Kuwait during the first Gulf War. After his military service he became the director of Swedish Family Medicine Residency in Seattle and pursued leadership positions within the education sector of family medicine in the US. While the president of Association of Family Medicine Residency Directors, he joined other leaders in family medicine to develop the strategic plan for family medicine nationally. As part of that strategic initiative, he joined the Primary Care Innovators Network and Converging Health which is focused on transformative payment and practice models of primary care to achieve better health care at lower cost.

With years of experience spanning inpatient care, rural outpatient clinic and complex care management nursing, Kathleen brings her experience into a key component of population health management by advising on best practices for care management of high risk patients.

Scott J. Thompson, COO
Mr. Scott J. Thompson has more than 30 years of business and technology experience, both as an entrepreneur in the population health management industry and as a technology consultant. As a health management industry executive, Scott was the co-founder, COO and Board member of Genetic Direction, a provider of genetics-based health management programs, and co-founder, COO and Board member of Catapult Health, which provides remote and worksite preventive health examinations to employer groups to enable the early detection and prevention of the most prevalent chronic health conditions in the United States. Prior to that, Scott also served as COO of Viverae (now Personify Health) and WinningHabits (now Optum), two leading population health management firms. Prior to his 25 years as a health management industry executive, Scott held senior consulting positions with Tactica Technology Group (now Hitachi Consulting) and Accenture where he specialized in technology solution development. Scott received a Bachelor of Business Administration (B.B.A.) in Management Information Systems from the University of Oklahoma.
