What is MyPHA?

MyPHA—short for My Personal Health Assistant—is a personalized advocacy and navigation program that guides individuals through the complexities of healthcare. Each participant is supported by a Personal Health Assistant (PHA), a concierge-like advocate and dedicated health coach, helping them understand their options and access the care they need, when they need it.

Purpose-Driven Personalized Support

The primary goal of MyPHA is to proactively engage high-risk individuals—identified via Vision Hawk™ and the Whole Person Risk Score—and guide them to the most appropriate care path. This might include Advanced Primary Care (APC), a specialist referral, or a relevant point solution.

Through high-touch support, MyPHA helps people take charge of their health, close care gaps, and achieve better outcomes while reducing costs.
